(00:01) Namaste. Welcome to Senior Consultant Dr. Sudi Kumar Neurologist Apollo Hospitals Jubilee Hills. Namaste sir. Namaste. Hello sir. Yes. Okay sir. Yes. general public. So main problems means headache almost 98% population and almost 50% population
(00:51) like migraine tension headache at least four patients with headache followed by headaches because of lifestyle. So the second maybe slip disc third maybe epilepsy quite common but treatment start they don't need to come frequently so and then emergency stroke paralysis out of entire life out of six person one person they will get paralysis so emergency basis So they are the main but
(01:39) okay neurologist. ologists. So what is the difference between neurologist and psychiatrist? >> Definitely important question because psychiatrist mean they don't want to go mental issues but brain health. So brain two
(02:24) specialist brain migraine epilepsy brain infection neurologist but psychological symptoms depression symptoms sadness depressed unnecessary anger symptoms then psychologist or psychiatrist okay psychologist psychiatrist difference but mainly suppose weakness and mental issues like anxiety, fear, sadness,
(03:10) neurosurgeon. Yes. Neurosurgeon. So second word surgery. So neurosurgeon operation. So brain spine. So for example, second brain fracture operations many headord tumor brain spinal tumors, accident,
(03:58) brain and spine injury problem and also Yes, definitely. Different types. So most common and then epilepsy common symptomologist because with treatment mostly medical
(04:44) treatment almost 65% epilepsy four epilepsy surgery like videogyp Okay. Yes. See most publiciz or focal epilepsy. Focal
(05:52) focal epilepsy. Third type body that is called focal onset. Secondary generaliz patient cannot give history because patient. So anystander family members so we can make out and then further testing treats. Okay. So important. So important
(06:39) question. So exactly first epilepsy type of epilepsy and third person family history infections like tape. So reasons 65% we can treat with medication important question. So suppose patient
(07:39) maxed. So starting we should not tell because brain ligong but persons within 3 years. So individual variations but first at least you know to minimum and then once we keep
(08:23) following them later on we can tell so that but maybe only in 20 30% where other patients li Okay. See t means [clears throat] benign tumor
(09:13) small size say 5 mm 10 mm size just for and then ask tumor detect that is called incidental tumor or incidental tumors if patients know symptoms then surgery but suppose patient already tumor symptoms start like headache start or depending on the location suppose Left side, right side face weakness shows right side.
(09:47) >> Right. Brain connection reverse only. Yes. >> Oh, sir. Okay. Okay. >> Correct. So, left side brain >> controls right side of face, right hand, right leg. So left side brain tumor or brain clotting condition right side other important function max% population left side lo so what you're telling understanding plus only person they are left-handed >> so they use everything their hand left so left-handed person right side only but All rightanded persons left. Okay.
(10:34) But suppose any suppose somebody gets injury, >> head injury or suppose stroke, brain clotting and by bad luck left side effect. >> So in addition to paralysis even okay >> right side effect speech will not be affected benefit. >> So best stroke avoid the best situation. Okay. But then we have to hope and pray normal.
(11:18) But left sideakis only one hand, one leg, one side. See stroke blood vessel blood supply blood. So brain left side right side. So almost always max right down on this brain stem. [clears throat]
(12:04) rare cases brain hemorrhage or clotting sometimes but you're right maximum 80% only one side effect most common stroke Look, maybe God's grace at least one side they can manage something and then one side walking
(13:17) See definite the stroke risk factors common risk factors number one high BP high BP high BP so BP high stroke number two high blood sugar other is diabetes. >> Oh okay. >> Number three cholesterol. So bad cholesterol LDL cholesterol triides again risk factor >> and then fourth sedentary lifestyle nowadays mut person 1 or two hours daily transport plus 8 to 10 hours sitting position.
(13:50) So sitting position more than 24 hours for brain stroke and then stress they taking housing loan generation 55 They used to think now 25 but because social med so stress different reasons plus
(14:37) relationship issues. So different different factors and then sleep hours for heart attack as well as brain stroke. So risk factors and then recent last two years. [snorts] So [clears throat] air pollution number one and then indirect effect and second brain stroke and heart attack. So in good journal
(15:28) high BP prevalence plus stroke [clears throat] only lungs but highpro connection heart and one factor global warming. So globing heat almost
(16:15) factor for stroke again. So neurology what is the reason neurology classical branch classical branch max and then and then tongue Yes. But neurology%
(17:08) mean to Yes. So common conditions migraine no only treats% egal so again treatment even family name so good in a way but taking diabetes blood sugarney
(18:08) department. So max departmentientology still classical branch and if directly if you show MR also same finding two okaycription months months Even medicopin treatment medicine
(19:24) therapeutic window narrow narrow means do and side effect do if I give high do starting low then patient side effect he will not tolerate the medicine >> and then starting they will lose the confidence sometimes warming so all these are not good side effects and then second reason so effects similarly same.
(20:15) Okay. And body gets used to it. But then similar starting effects. So to stop the side effects. Okay. Patient mindset. So
(21:15) Parkinson disease progression. Okay. So do increase same dose same medicine dose increase or sometimes addative conditions like they will know also So condition they can still manage well and then
(22:04) deep brain stimulation. Okay. So suppose medicine deep brain stimulation yes starting so 20 to 30% % 20 30% but 60 70
(22:51) [clears throat] additional findings by Earth years treat symptoms because sudden got lost. There's something called aura. Aura
(23:37) means sometimes most common triggering factors like suppose number one medicine. Suppose somebody already taking medicine again. Second sleep deprivation. So late night functions on the marriage party or some five hours
(24:22) next. And third the fasting because brain health function. Yes sir. Max correct light but complete fasting epilepsy person avoid headaches. Headaches vomiting sensation
(25:14) they can't stand and they can't concentrate also headache Fore consultation important topic because migraine or headaches very common almost young persons low 15 20% persons sometimes migraine var and almost to% young population 15 to 35
(26:02) 40 symptoms connected to migraine number one symptomig okay and then migrant minimum 4 hours up to 72 hours. Second 4 hours up to 72 hours full headaches migraines along with headache four additional symptoms. Number one nausea. Nausea means feeling of
(26:47) woundings but sensations. Number two symptom warming actual womings. [clears throat] >> Number three phototo phobia. Photophobia is better. Phonophobia. Phonophobia means any sound. They don't like sound. Even headphone even talking to family members. Yes sir. Yes. >> So any symptoms migrated. You you also mentioned two additional symptoms important number one number one vision symptoms.
(27:24) So vision symptoms that is called visual aura migrant symptom. So sometimes sometimes shoulder pain and second symptom that is called migraine or migr all these are symptoiger painillers. Yes sir.
(28:08) >> Shall we give or not? Then we can use something personally on one time. Okay. >> For two reasons. Number one side effects right from stomach acidity ulcer sometimes kidney damage liver damage even heart so effects second and most important [snorts] okay
(29:00) >> so that the person doesn't need painkillers. So thenologist preventive treatment. Okay. next serious because prolonged stress work condition overarch
(30:05) heart attack and stroke risk. So 55 means India max 5 days or 6 days. >> Yes sir. >> So 5 days more than 11 hours per day >> 6 day week persons if they do 9 hours work also per day 55. >> So overwork and stress at work they're not good for health. So serious side effects. So heart attacks and stroke working hours 40 to 45 hours maximum per weekly.
(30:59) But farmers are the only day >> farming communitys 6:00 6:30 dinner by 9:00. >> Okay. Their habits are healthy. >> Correct. Sitting not good for health actually not relaxing. If you search and Google sitting is the new smoking. So sitting smoking smoking similar risk factor sitting. So sitting for more than say 10 to 12 hours per day job 8 to 10 hour 8 to 9 hours job sitting and then transport 1 to two hours sitting position and then person
(31:46) maximum work You cannot do anything 30 to 45 minutes 2 minutes. So they should leave the chair office you can sit and similarly coffee or any tea breaks if they cannot go out of the office. chair they can leave the chair and 10
(32:31) times >> easy easy also >> easy also because no need to go outside >> no need to go outside no need to invest anything just 10 minutes >> and all these things what I'm telling research office 8 to 9 hours sitting risk of overweight sugar BP BP, high BP, high sugar. So frequent proper treatathy means.
(33:34) same if not treated then that is called neuropathy act but short neuropathy >> and then common reason diabetes because diabetes almost adults 40 persons pre-diabetes or diabetes only and Common complication diabetes maximally sugars are not control and then we can make them comfortable related to neurology. Yes. Diabetes
(34:26) >> neurology symptoms. So nerves two types nerves. One nerve called sensory nerves. Sensing sensation when I touch nerves are the motor nerves. Motors neuropathy. Sometimes sometimes motors effect cannot stand properly. walking problem
(35:19) stairs problem. >> Okay. >> As I told different reasons but others vasculitis vasculitis means blood supply infection called leprosy. But still tingling same sensory nerves effect tingling pins and needle sensation tingling tingling sensing shaking resting sitting
(36:11) one side either left or right. Okay. So then steps we should think of Parkinson's. Okay. Tingling are the symptom of neuropathy. >> Neuropathy. Okay. There is a difference. >> Correct. Correct. Yes. So they have to continue.
(37:08) See Parkinson's can happen at any age but old person and 10% 15% cases family history so because of genetic defect it can happen at early age but yes okay even like year by year symptoms. Okay. >> But yes, deep brain stimulation. >> So different different options. Okay. So at least good 15 to 20 years
(37:54) good quality of life but yesong will be there again then they have loss of balance 15 but multitation can happen. >> Oh okay. Correct. But medication daily to two or three tablets. Oh, nice.
(38:54) sir brain problem seems brain but even spinal cord so nerves so brain problem yes problem brain problem but nerves are there in entire body different different body parts okay >> so problem any body part can be affected not only brain but any part so brain only brainffect No other additional brain, spinal cord.
(39:23) Yes. >> Optic nerve, hand problem can affect any part of body. >> Okay. So this is myth. >> Myth. Correct. >> Okay. Brain. Is it true or false? It is true for adults [clears throat] and older persons. So which means one year baby and sometimes 8 years only >> but suppose 20 year 30 year old persons or old persons age okay so it's completely dependent Okay. Stroke.
(40:17) Yes. But one person compared to normal healthy persons risk recurrence. So that definitely lielong blood thinner and statin risk factor control because if you don't use then stroke recurrence so benefits more and okay liong blood like bleeding risk. Yes sir.
(41:24) We can manage hormonal functions. This is why the my policy is the fun is prevention first ever stroke itself prevent >> don't wait for the first stroke >> first stroke prevent because risk factors factors control almost 80% strokes can be prevented prevention is better than yesrobuster therapy time window 4 and a half hours 80% Okay.
(42:07) So first risk of disability is high. [clears throat] So max first stroke itself prevent the best option. >> Okay. Uh one more thing sir uh physiootherapy will help Parkinson's >> definitely because physiootherapy means that the exercise plus other now different different instruments machin I told they have difficulty in walking cyclingary bikes And then chair something to standing
(42:58) five to 10 times that also is good for leg strength and balance. >> Okay. Contraction simple walking. So walking physotherapy definitely improvement for almost all neuro issues. Parkinson's key, stroke >> and even paris due to accidents, injuries, spinal cord injuries. >> So physotherapy early start. >> Okay. >> If you delay also then again starting definitely beneficial.
(43:30) >> Okay. Uh we use only 10% of brain sir. >> No no [laughter] that is that is a myth only. So each part of brain are the different functions. >> Okay. So we use all the brain >> all the brain >> even the people also use all the brain and those who are not in if see how can we use 10% if I move my left hand then right side brain activate >> okay >> if I move right hand then left [snorts] side brain activate so at least everyone >> definitely >> so definitely left side plus right side use we are speaking speaking left part
(44:06) activate yes >> vision vision hospital. So almost if somebody is using you know mouth they speaking vision hearing hand% cover >> then emotions without emotions systemic system. So everybody has memory. So, so that means so every person using 100% of brain not even 90 >> brain damage is always permanent
(44:51) >> brain damage depends on the cause. So suppose injury head injury head injury brain damage >> or stroke brain damage >> and severity brain damage there is something called neuroplasticity. So recovery brain function recovery surrounding parts or other parties you know 20 30% of brain effect but suppose one or two
(45:37) restore. >> Okay. >> Then complete recovery. >> So duration also matters. >> Duration is matter. >> Suppose somebody attempts hanging >> again. Somebody has noted and brings down. So patient oxygen supply again. But suppose somebody complete hanging. So duration important function.
(46:26) Correct. See first different different organs different different healthy foods are just more of the gimmicks. >> Okay. If you can see number one means converts to sugares over diabetes. [clears throat] So carboid restriction body parts help. Okay. Second principle proteins because mostly our vegetarian diet. So
(47:13) vegetarian protein means d pul paneer dairy items. So unless we pay full attention proteins definitely. >> So normal adults at least 1 to 1.5 1.6 6 kg bodygrenus good muscles mass and muscle strength not only for appearance muscle if we eat carbohydrate glucose it can be better utilized [cough] so again risk of diabetes overweight and plus risk of falls old so that is second principle >> third principle the essential vitamins So brain specific vitamin B12 B12
(48:00) >> and thin important. So B12 again plant foods B12. >> Oh okay. So vegetarian who are eating animal sourced foods like paneer items of B12 but specific amount. So that is a veget and then most likely those who are alcoholics. So alcohol deficiency risk. Okay. And then see brain health they saying you know small handful of nuts nuts like almonds, cashews, pistachio.
(48:47) Correct. Not because of shapey good proteins and then >> healthy fats essential vitamins minerals. So, nuts, kulpi, handful or 25 to 30 grams good for brain health, heart health also and then seeds because >> [clears throat] >> omega seeds and then the sea fish like salmon. So all body parts help not only correct correct and what we can sustain.
(49:35) Okay. Again somebody says it coming from strawberries, blueberries even peanuts. So peanuts easily available. So small handful of peanut no problem good source of calories. Yes. So portion even if sometimes they put lot of salt salted almonds salted pistachio but too much salt
(50:20) BP salt restrict healthy persons normal salt. >> So these are some of the healthy foods. >> Okay. So carbohydrates, proteins, vitamins there is no food for any specific organ. I think that is people want to know like one thing they will say liver detox not all this thing wellness clinics liver body detox kidney detox liver detox there's nothing like that >> see our body itself mechanism only >> kidney round the clock function detox so when our healthy person detox food they'll say some three juices only
(51:05) [clears throat] carrot this that beetroot if you drink it body detox at it may be harmful also because concentrated juice can some people cannot digest also >> so whole fruits always better >> okay fruits >> compared to fruits fruit juices >> packaged fruit juices max >> because [clears throat] packaged fruit so that everybody should avoid fresh fruit juice but sometimes we can consume more because of this whole fruits you cannot eat more than one apple. One apple already satisfied.
(51:47) So compared to fruit juices, whole fruits better. It's limitless because it is it is satisfying fully filling juice four mindful not 50% but if your gut digestion definitely mouth Start we have enzymes. Yes. Correct. So
(52:33) eating 15 20 minutes sometimes 5 minutes. So that is not good. There is no good. Just imagine each specific we have to eat so many food basil. Correct.
(53:17) Second entry are the packaged foods. >> So that is called ultrarocessed packaged foods. So chips say >> all those are unhealthy. Secondly, energy drink, energy drinks, soft drinks, sugar, very high sugar. So max. So what to avoid and then what to eat. So that is beneficial. Correct. Correct.
(53:48) Thank you so much for sharing your knowledge. Thanks. Thank you. I also enjoyed. >> Thank you, sir. Pleasant and much discussion. >> Thank you. Thank you, sir. Thank you so much.
